Welcome to the MHBP online Preferred Provider Directory. This directory is designed to help you easily locate hospitals, physicians, pharmacies and other types of health care providers participating in the MHBP Preferred Provider Network.

Service Provider Request
To request that a service provider be added to this list, please download the Provider Request Form. To request that a service provider’s address or tax ID information be changed, please fill out and return the Provider Change Form.

While we take great care to ensure all information is correct and complete, the network of participating providers is subject to changes from time to time. Please note that a provider may not choose to participate in the network. You should always verify that a provider is participating in the MHBP PPO Network before receiving services by calling the provider directly or contacting MHBP Provider Relations at 501-978-6137. You may also email [email protected]. Credentialing of participating providers is delegated to the individual, clinic, hospital and/or contracting entity. Providers are listed solely because they have agreed to certain financial and utilization criteria.
